Once you have reviewed the Manual and the Video, please complete the documents below and email them to HousingServices@KernBHRS.org.

Additional Documents are required to determine the capacity of your facility without obtaining a Conditional Use Permit (CUP). Please complete the CUP verification form from the city or the county depending on the zoning area where your facility is located. 

 If you already have a facility address, you can take the CUP Verification Form to the City or County agency that has zoning jurisdiction for the property address. Please complete the top section of the form, and request the planning department complete and sign the bottom portion, identifying the maximum capacity allowed by the agency's zoning rules without a Conditional Use Permit. Once completed, please email the form to HousingServices@KernBHRS.org.
